頚部放射線照射後の頚動脈狭窄に対してproximalおよびdistal protectionを併用し頚動脈ステント留置術を施行した1例

Ⅰ．はじめに

　頭頚部の悪性腫瘍に対する放射線照射後に高度の血管狭窄が起こることが知られている．放射線照射後の頚動脈狭窄症（radiation-induced carotid stenosis：RI-CS）は，近年癌の予後が改善し，長期生存例における脳梗塞の発症の一因として問題となっている．RI-CSは放射線照射領域に一致して数年の経過とともに狭窄病変が進行し，不安定プラークを呈することが多く，十数年後に脳梗塞を発症する危険性が高い病態である^{2,3,6,7,11-13)}．RI-CSは脳卒中治療ガイドライン（2009）では頚動脈内膜剝離術（carotid endarterectomy：CEA）危険因子とされ，頚動脈ステント留置術（carotid artery stenting：CAS）を行うことがGrade Bとされている．今回われわれは，放射線照射後の頚動脈狭窄症に対してproximalおよびdistal protectionを併用してCASを施行し，術後経過が良好であったので報告する．
