頭蓋頚椎移行部は解剖学的，生理学的，臨床的に以下のような特徴がある．
（1）解剖学的：重要器官が集中（頚・椎骨動脈，脊髄・下位脳神経，咽頭），独特な骨構造
＝立体構造理解が容易でない
（2）生理学的：重い頭部の支持と重要器官保護⇔自由な回旋運動の主軸
＝相反する機能を担うため，手術適応・術式決定が難しいことがある
（3）臨床的：脳神経外科・整形外科・耳鼻咽喉科など，複数の科にまたがる領域
＝お互いに技術・知識の壁があり，連携診療が求められる
本稿では脳神経外科・脊髄外科の観点から，頭蓋頚椎移行部外科に関し成書からは容易に学べないところを意識して説明する．
