真のmixed pial-dural arteriovenous malformationの1例

Ⅰ．はじめに

　Mixed pial-dural arteriovenous malformation（mixed pial-dural AVM）はNewtonとCronqvistにより1969年に提唱された概念であり，硬膜動脈と軟膜動脈の両者がfeederとしてnidusに流入するAVMと定義されている⁵⁾．この分類はAVMのfeederの種類に基づくものであり，nidusが硬膜と脳実質内の両方に存在することを必ずしも意味しない．近年では，硬膜動脈が脳表に近いpial AVMに流入することは稀ではないことが明らかになっている³⁾．一方で，nidusそのものが硬膜表面と脳実質内の両方に存在する，いわば「真のmixed pial-dural AVM」の報告は，検査手法や顕微鏡手術が進歩した現在においても非常に稀である．

　われわれは小脳テントと脳実質内の両者にnidusが存在するmixed pial-dural AVMの1例を経験し，術中所見と病理所見の詳細な検討を行い得たため，文献的考察を加えて報告する．
