Ⅰ．はじめに

　頚動脈内膜剝離術（carotid endarterectomy:CEA）の周術期合併症のなかで，術後の脳・頚神経麻痺は一過性のものも含めると1.7～42.0%と最も頻度が高く，しかも頚動脈ステント留置術にはないCEA固有の合併症でもあり，いかに回避するかは重要な課題である^{2,4,5,8,10-14,16,17,19,21,24-28,30-33,36)}．本稿では，CEA術野における脳・頚神経の局所外科解剖および神経損傷回避のための手術手技の要点について，自験解剖標本（Fig.1～12），自験手術例（Fig.13～15）を提示して述べる．
