CAS後ステント内狭窄にstent-in-stentingを施行した2例の検討

Ⅰ．はじめに
　頚動脈狭窄に対する頚動脈ステント留置術（carotid artery stenting：CAS）は，頚動脈内膜剝離術（carotid endarterectomy：CEA）とならび，標準的な治療手段となってきている．どちらの治療法でも再発は起こり得るが，CAS後の再狭窄，閉塞は2年で6〜12%程度に起こるとも報告されている³⁾．症候性または高度再狭窄には再治療が行われ，血管内治療が有利であるが，周術期の脳梗塞や長期成績が問題である．今回，CAS後の内頚動脈狭窄に対し，stent-in-stentingを施行し，その後2年以上patencyの維持に成功した2例を経験したので文献的考察も加えて報告する．
