Ⅰ．はじめに
　側頭葉内側前方病変の摘出方法は，前方もしくは側方からのアプローチが一般的である．病変が海馬あるいは海馬傍回に浸潤し，これらの脳回構造に沿って後方へと発育した場合，脳弓においては視床枕後方部方向へ，また，海馬傍回においては，帯状回後部や楔前部方向へと進展する．こうした側頭葉内側病変で，その後方部分が高位に進展するような病変を，前方および側方から安全かつ一期的に摘出することは難しい．今回，側頭葉内側後方を主座とし後方部分が高位に進展するgliomaの摘出術において，ナビゲーションと術中MRIを用いたsupratentorial-infraoccipital approachが有効であった2例を経験したので文献的考察を加えて報告する．
