てんかんと高周波律動

近年,高速サンプリングが可能なデジタル脳波計が出現したことで,80〜500Hzの高周波律動(high-frequency oscillation:HFO)の分析が可能になった.HFOとは,あることを集中して考える時(生理的HFO)やてんかんの発作(病的HFO)などの際にいくつかの神経細胞が集団で同期して活動(クラスター)するために現れるかなり大きな律動波を記録した脳波である.このうち病的HFOは新たなてんかんバイオマーカーとして注目を集めていて,焦点診断に従来から用いられていたスパイクよりもてんかん原性領域の同定に有用であることが示唆されている17).本稿では,HFOについて概説し,てんかん治療における病的HFO解析の有用性について解説したい.
