下垂体腺腫に対する蝶形骨洞手術への内視鏡導入以来7,8,24),トルコ鞍周辺頭蓋底の内視鏡的解剖知見の集積や経鼻内視鏡手術に特化した器具の開発などとともに内視鏡による広角視野および斜視機能を活用した拡大蝶形骨洞手術が行われるようになってきた2,16).一口に拡大蝶形骨洞手術と言っても,前方は前頭蓋底から尾側は斜台部病変,さらに側面は海綿静脈洞病変などを対象とする手術アプローチを含むが,本稿では蝶形骨平面から鞍結節経由で傍鞍部腫瘍に到達する手術手技に焦点を絞り,その手術戦略・手技について傍鞍部周辺の解剖所見を中心に解説する.
