Four hands techniqueを用いた経鼻内視鏡頭蓋底手術

Ⅰ．はじめに
　内視鏡手術の進歩に伴い，脳神経外科においても経鼻内視鏡手術が普及しつつあり，顕微鏡下では到達困難な部位まで操作可能であることから，手術対象がトルコ鞍部の下垂体腫瘍から頭蓋底疾患へ拡大している．実際に，経鼻内視鏡頭蓋底手術（endoscopic endonasal skull base surgery：EESS）により，従来の手術法と比較して低侵襲でかつ良好な治療成績の報告例が増えている．一方，EESSは，これまでとは異なる外科解剖や新たな手術手技の習得が必要である．従来は，内視鏡を固定した経鼻手術が一般的であったが，内視鏡をフリーハンドで操作するfour hands techniqueにより操作性が向上し，さらに経鼻内視鏡手術に習熟した耳鼻科医とのチーム手術は，EESSの安全性および治療成績向上に貢献している．
