脳性麻痺に対するバクロフェン髄腔内投与療法

バクロフェン髄腔内投与（intrathecal baclofen：ITB）療法が本邦で保険適用となってから約9年が経過し，その効果については広く知られるところとなっている．ITB療法は，GABA-B受容体のアゴニストであるバクロフェンを体内植え込み型のポンプを用いて持続的に髄腔内に投与する治療法である．痙性を伴う麻痺や緊張性の運動異常症に対して，その症状を緩和させることにより，運動機能やADLの改善を得ることができる．バクロフェンは経口投与では眠気などの副作用により十分な薬理効果を示さないことが多いが，髄腔内に投与することで，経口の100分の1程度の投与量でも劇的な痙性の改善を得ることができる．本邦でも1,000例以上の患者に対してITB療法が施行されており，これらの疾患に対する治療法として重要な位置を確立している．その中でも脳性麻痺は脊髄損傷後の痙縮や痙性対麻痺とならんで数多いITB療法の適応疾患である．本稿では脳性麻痺に対するITB療法の概略，手術，効果，合併症，維持方法などについて最近の知見を交えて詳説する．
