Ⅰ.はじめに
 内視鏡手術の進歩に伴い,経鼻手術の対象がトルコ鞍近傍から頭蓋底疾患へ拡大している.特に斜台は従来の手術法では到達困難な部位であったが,経鼻内視鏡手術により,低侵襲にアプローチ可能となった.経鼻手術では,鼻腔,副鼻腔を含め術野展開の予測が困難な場合があり,術前のシミュレーションが重要である.本稿ではトルコ鞍から斜台近傍腫瘍に対する経鼻内視鏡手術について,手術支援装置と手術の基本ステップに沿って要点を述べ,さらに代表的な疾患について概説する.
