Bath plug closure法を用いた経鼻内視鏡的髄液漏閉鎖術—耳鼻科・脳神経外科連携領域の治療

Ⅰ．はじめに
　髄液鼻漏は，頭蓋底の骨や硬膜およびくも膜の連続性が破綻することで，脳脊髄液が頭蓋内から鼻内へ漏出する病態である．その要因はさまざまであるが，外傷性と非外傷性髄液鼻漏に大きく分類され，後者には頭蓋底手術に伴う医原性の病態や髄膜脳瘤，髄膜瘤などが含まれる．
　近年の内視鏡技術の発展に伴い，髄液鼻漏に対する外科的治療は経鼻内視鏡的アプローチが選択される場合が多い^4,10,20,21)．その術式は，瘻孔の大きさに応じて異なる閉鎖法が選択される．前頭蓋底腫瘍切除後のように，意図的に硬膜を切除する場合は欠損範囲が広いため，遊離筋膜および鼻中隔粘膜を用いたmultilayer closure（多層性閉鎖）を要するが，硬膜損傷が断裂程度の小さな損傷である場合は，onlayでのsingle layer closure（単層性閉鎖）で治癒が期待できることもある¹³⁾．その一方で，瘻孔が中等度であり，遊離筋膜を硬膜内に敷き込んで縫合するほどの大きさではない場合，bath plug closure（フロ栓閉鎖）法が有効であると報告されている^20,21)．硬膜および硬膜内操作を要する手技ではあるが，副鼻腔手術の延長として耳鼻科医によって施行されることが多く，本法に関する脳神経外科医の認識は十分とは言えない．今回われわれは，当院における経験を通して，髄液鼻漏に対するbath plug closure法の実際について報告する．
