Ⅰ.はじめに
 術中運動誘発電位(motor evoked potential:MEP)モニタリングは,現在の脳神経外科手術における術中電気生理モニタリングの中で,最も重要な役割を担っている.MEPがモニタリングする皮質脊髄路は四肢の随意運動を担っており,その機能障害は日常生活機能の低下と直結しているからである.
 本稿では,術中MEPモニタリングの現状と諸問題を提示する.実際にはこれらの問題すべてが有害というわけではなく,さほど気にせず行われていると思う.また,今回提示する問題点を解決する方法はなく,最適解を出すことも難しいのが現状である.脊髄手術におけるMEPと頭蓋内病変におけるMEPは発想や求めるものが異なり,同一のモニタリング手技として考えることは困難であるため,本稿では頭蓋内病変に対するMEPモニタリングを主として記載することとする.
