内視鏡によるkeyhole transcranial approachのバリエーション

Ⅰ．はじめに
　内視鏡手術技術，器具の発達に伴い，脳神経外科領域においては経鼻手術をはじめさまざまな術式が開発され普及しつつある．その最大の特徴は，小さな入り口から入り，広い視野を観察できる点である．一方，顕微鏡下の低侵襲keyhole手術のコンセプトが導入され，supraorbital approachをはじめ，必要最小限の皮膚切開，開頭からアプローチする方法が普及した⁸⁾．この考えに内視鏡の要素を合わせると，限られた入り口からより広い範囲の手術野を得られることになる．斜視鏡の使用で顕微鏡では見えない領域の操作が可能になり，こうした取り組みも報告されている^1-4,9)．自在に正確な固定ができるホルダーの使用と，近年登場したexoscopeを応用し，endoscopeと組み合わせることにより顕微鏡手術，内視鏡手術両方の要素をもった手術がシームレスに可能になる．また，われわれはこのアプローチを内視鏡下経鼻手術と同時に行う取り組みも行っている⁶⁾．
　本稿では，脳実質内腫瘍，脳室内腫瘍を除く，頭蓋内脳実質外腫瘍に対する内視鏡単独手術を中心に，さまざまな内視鏡下keyholeアプローチについてまとめた．
