神経集中治療(neurocritical careあるいはneurointensive care)の目的は,神経学の知識を背景とし,さまざまな侵襲的・非侵襲的モニタリングを駆使して頭蓋内環境を最適化することにより,脳損傷を最小限にとどめて重篤脳損傷患者の転帰改善を図ることにある39).このため,担当医—neurointensivist—は脳神経と全身臓器の病態生理に精通し,関連各職種を統括して集学的治療を行う30).傷害された脳の病態・生理に即した呼吸・循環・代謝の全身管理は,脳指向型集中治療と換言できる34,37).逆に,脳損傷がもたらす全身への影響も看過してはならない29).
