（4）高齢者頭部外傷治療におけるピットフォール

Ⅰ．はじめに
　本邦では高齢化が急速に進展しており，医療の分野においても未曽有の超高齢社会に対応することが喫緊の課題となっている．日本脳神経外傷学会の「日本頭部外傷データバンク」をみてみると，例に漏れず日本の頭部外傷患者における高齢者の割合が増加しており，最新のプロジェクト2015（2015-2017年登録）では，重症頭部外傷患者の半数以上が65歳以上の高齢者であった．日本で頭部外傷診療を行う上で高齢者頭部外傷は稀な存在ではなく，日常的に遭遇する患者群である．高齢者は，若年者とは異なる解剖学的・生理学的特徴や病態を有する．これまでの報告では，高齢者頭部外傷の転帰は若年者と比較して悪いとされている^9,27)．しかし，高齢者頭部外傷に特有の病態を理解して診療にあたれば，治療におけるピットフォールに陥ることなく，転帰を改善する余地があると考えられる．
　本稿では，これまでの高齢者頭部外傷治療の疫学的調査結果を検討しながら，高齢者頭部外傷に特有な病態を解説する．さらに，高齢者頭部外傷の病態を踏まえた上で治療戦略を考察する．読者の方々の今後の日常診療に役立てていただきたい．
