太田母斑を併発した頭蓋内原発テント上meningeal melanomaの1例

Ⅰ．はじめに
　中枢神経系原発の黒色腫系腫瘍は，脳軟膜のメラノサイトから発生する稀な腫瘍である．このメラノサイトは脳幹から脊髄に高密度に分布しているため，原発性黒色腫系腫瘍は後頭蓋窩や上位頚髄での発生が多く報告され，テント上の腫瘤としての報告は稀である¹⁴⁾．一方，太田母斑は不規則な顔面や強膜の変色で，多くは片側性で三叉神経の分布域に一致している^6,14)．この太田母斑が悪性化することはほとんどないとされている¹⁴⁾．しかし，太田母斑とneural crest（神経堤）に由来する臓器に腫瘍病変が併発することが知られており，頭蓋内原発の黒色腫系腫瘍と太田母斑（皮膚・眼メラノーシス）の関連性を推察する報告もある^{1,6-9,15-17）}．今回われわれは，頭蓋内原発でテント上に腫瘤を形成したmeningeal melanomaを経験したので，文献的考察を加えて報告する．
