Ⅰ．経験症例
　〈患　者〉　62歳　女性
　現病歴　3カ月前に生体弁を用いた大動脈弁置換術が前医で行われており，ワルファリンを1日2.5mg内服中であった．2015年某日，8：15に左上下肢麻痺・構音障害にて発症した．前医救急搬送となり，MRIにて右内頚動脈（internal carotid artery：ICA）閉塞の診断となった．Prothrombin time-international normalized ratio（PT-INR）が2.02であったためtPA静注療法は施行せず，血栓回収療法を目的に当院に転院搬送となった．
