開頭術で生じた綿片遺残を経過観察とした2症例：回収手術の是非

Ⅰ．経験症例
　〈患者1〉63歳　女性
　めまいの精査で発見された多発脳動脈瘤（前交通動脈，左内頚-後交通動脈，左中大脳動脈）の経過観察中に，くも膜下出血（Grade Ⅳ）を発症した．前交通動脈瘤を破裂部位とみなし，左pterional approachを施行した．くも膜下血腫が多く，脳は腫脹していたが，脳室ドレナージ下に前交通動脈瘤へのクリッピング，左中大脳動脈瘤へのクリッピングとラッピング（酸化セルロース綿をフィブリン糊にて固定），左内頚-後交通動脈瘤へのラッピングを終えた．脳表に敷いていた保護シートを除去すると綿片（Delicot 5mm×8mm：American Surgical Company, Massachusetts, USA）の黄色い識別糸がシルビウス裂の中大脳動脈瘤クリップより遠位部から出ているのが確認され（Fig.1A），同糸を深部に辿ったが中大脳動脈瘤ラッピングに用いたフィブリン糊と血腫で組織が剝離できず，綿片本体は発見できなかった．優位半球へのダメージが危惧されたため，綿片回収を断念し，識別糸を切断して閉頭した．
