末梢神経鞘腫の手術

Ⅰ．はじめに
　神経原性腫瘍（neurogenic tumor）は，神経線維に由来するものと神経節に由来するものに大きく分けられるが，神経線維に由来する腫瘍としては神経線維腫（neurofibroma）と神経鞘腫（schwannoma）がある．それぞれ大半は良性であるが，悪性腫瘍も存在する．
　現時点では，神経原性腫瘍の手術をどの臨床科が扱っているのかが非常にわかりにくい状況になっている．皮下軟部腫瘍との判断で手術を行えば，間違いなく神経症状を来すため，医療紛争になりやすい．
　本稿で取り上げるのは良性の末梢神経鞘腫であるが，他の神経原性腫瘍と比べてほとんど神経症状を来すことなく摘出手術が可能である．そのためには，発生母地と腫瘍と周囲構造物の関係を理解することが重要である．脳神経外科医は，頭蓋内・脊髄腫瘍としての神経鞘腫の外科治療を十分に行っているため，本来は末梢神経鞘腫も扱うべきではあるが，残念ながら日本の脳神経外科医は脳以外に興味を示さない傾向があり，経験もないため手術に考えが及ばないのが現状のようである．しかし，末梢神経鞘腫においては，周囲に危険構造物が少ない上，神経自体の構造を知ることができる絶好の手術手技である．
　本稿では，末梢神経鞘腫の手術は，神経を愛護的に扱うことを意識できる基本手技であることを示したい．
