脊髄未熟奇形腫の合併を術前画像診断した髄液漏を伴う先天性皮膚洞の1新生児手術例

Ⅰ．はじめに
　脊髄奇形腫は全脊髄腫瘍の0.2〜0.5%とされる稀な腫瘍である^3-5,13)．脊髄奇形腫が胎生早期に発生した場合，その発生過程により神経管癒合不全を高い頻度で合併すると報告されている¹²⁾．新生児例においては，脊髄髄膜瘤などの神経管癒合不全として手術を行い，術後標本の病理学的所見から後方視的に奇形腫の併存であると確定診断された症例も散見される⁶⁾．緊急手術を要さず手術を待機している神経管癒合不全においては，その成長過程で脊髄奇形腫の合併が画像診断されることはあるが，新生児期に緊急手術を要する場合は，新生児という条件があり，術前の画像検査で奇形腫の合併を診断できることは稀である．
　今回われわれは，先天性皮膚洞を合併した脊髄未熟奇形腫の1新生児例を経験した．本症例では，先天性皮膚洞より髄液漏を認めたため緊急手術を要したが，術前画像診断で脊髄未熟奇形腫の存在を確認していた．本報告では，本病態に対する治療方針を中心に，文献的考察を加えて報告する．
