Ⅰ．はじめに
　平均寿命が延びて超高齢社会になるにつれて骨粗鬆症性椎体骨折（osteoporotic vertebral fracture：OVF）の症例が増えている．従来，脳神経外科医がOVFの治療に携わる機会は少なかったが，近年腰椎を含めた脊椎手術を行う脳神経外科医が増えており，今後はOVFを伴う脊椎症例を診療する機会が増えると考えられる．本稿では神経脱落症状を呈する胸腰椎移行部のOVF症例に対する外科治療，特に後方除圧固定術と胸腰椎側方椎体置換術を中心に述べるが，治療に必要な病態や診断についても概説する．また，誌幅の都合で本稿では割愛するが，実際の治療に際してはOVFの保存的治療や骨粗鬆症の診断・治療の知識が必須であり，成書を参照していただきたい．
