超急性期脳梗塞に対する再開通療法の現在

Ⅰ．はじめに
　アルテプラーゼを用いた静注血栓溶解療法（intravenous recombinant tissue plasminogen activator：IV rt-PA）は，わが国では2005年に認可され広く行われているが，2010年のMerciリトリーバー（Stryker, Kalamazoo, USA）導入以降行われるようになった血栓回収療法は，新規デバイスを用いた複数のランダム化比較試験（randomized controlled trial：RCT）によりIV rt-PAを含む内科治療に優る転帰改善効果が証明され，今や標準治療となった．本稿では，超急性期脳梗塞の再開通療法について，現在までのエビデンスを俯瞰し，現状と課題を述べていきたい．
