第四脳室出口閉塞症候群に対するシャント術後遅発性合併症における緊急シャントクランプシステムの有用性—2症例報告

Ⅰ．はじめに
　第四脳室出口閉塞（fourth ventricle outlet obstruction：FVOO）は，小脳出血後，脳腫瘍，キアリ奇形など種々の原因により，ルシュカ孔，マジャンディー孔の部分—完全閉塞を来し水頭症を併発する．FVOOによる水頭症に対しては内視鏡的第三脳室開窓術（endoscopic third ventriculostomy：ETV）が第一選択である．しかし，急性期にはFVOOの確定診断は容易でなく，さらに，病因によっては水頭症の発生機序に髄液吸収障害が関与すると予測されることも多く，脳室腹腔シャント術も選択されることがある．今回われわれは，FVOOの関与があったと思われる二次性水頭症に対して急性期に脳室腹腔シャント術を行い，遅発性に髄液排出過多，slit ventricle syndrome（SVS）に至った症例を経験したので報告し，FVOOが関与する二次性水頭症に対する治療方針について考察する．
