CFDによる動脈瘤の壁の厚さに関する検討—Translucent typeの未破裂中大脳動脈瘤は術前に予測できるか?

Ⅰ.はじめに
 画像診断の進歩に伴い未破裂脳動脈瘤の検出率が高くなり,治療件数が増えている.動脈瘤の大きさや部位,形状などといった破裂リスク23,26,27)をもとに,その治療が考慮されている.Blebもリスクの1つとされ,blebの部分では壁が薄いとされることが多い28).未破裂動脈瘤の術中所見として,壁が非常に薄く,中の血液が透見されるような症例は稀ではなく,壁の薄いところが破裂点であるとする報告1,5,11)もあり,動脈瘤壁の性状が破裂リスクの1つではないかと考えられてきた.しかし,これまで壁の厚さを術前に評価することは困難であった.
 近年,高解像度のMRIやcomputed fluid dynamics(CFD)解析を用いて壁の薄い部分を評価した報告2,4,7,10,13,25)が散見される.しかし,これまでの報告は,いずれも動脈瘤の一部分の壁の厚さを評価したものであり,動脈瘤全体の壁の厚さに対する画像所見の報告は,渉猟した限り認められなかった.今回われわれは,動脈瘤壁のほぼ全体が薄い症例を術前に評価することが可能か,CFD解析を用いて検討を行ったので報告する.
