内視鏡を用いた新たな手術アプローチへの挑戦—髄膜腫に対する内視鏡下鍵穴手術，高齢者での「脳を空気に触れさせない」手術

Ⅰ．はじめに
　手技の精密さやアプローチの多彩さにおいて開頭顕微鏡手術の完成度は非常に高いものの，あくまで頭蓋外に置いた光源と視点によって観察しているために，深部での視認性にはおのずと限界がある．
　内視鏡手術は，「頭蓋内（＝病変の近傍）に視点を置く」という発想の転換によって，小さい入口部からでも深部で明るい広角視野を得ることを可能とした．また，視軸の自由度が高いことによる体位制限の少なさや水中手術などの特性も，多くの脳腫瘍の手術にブレイクスルーと言える違いをもたらすものである．
　内視鏡下の腫瘍摘出術にはさまざまな手法があるが，本稿では髄膜腫に対する小開頭での内視鏡下摘出術について，そのコンセプトや手法を解説していく．また，それを応用して高齢者に対し行っている「脳を空気に触れさせない手術」を紹介する．小開頭は顕微鏡手術では忌避されがちだが，内視鏡手術とは非常に相性がよく，患者の利益に寄与するところの大きい手法である．本稿が普及の一助となれば幸いである．
