Ⅰ．はじめに
　てんかんは，有病率約0.4〜0.8％と頻度の高い神経疾患である．また，近年では高齢発症例や脳卒中後てんかんも増加傾向にある．脳神経外科領域においては，脳腫瘍や手術後に症候性てんかんを合併することも多い．抗てんかん薬による薬物療法はてんかん治療の中心であり，一般的には抗てんかん薬により約70％の患者は寛解状態となる．近年は新規抗てんかん薬の出現により，治療の選択肢もさらに増加しており，成人てんかん患者の薬物療法についての理解は，脳神経外科領域においても重要事項の1つである．
　本稿ではまず総論として，2018年に日本神経学会から出版された『てんかん診療ガイドライン2018』¹⁰⁾の内容を中心に，成人てんかんの薬物療法について述べる．その次に，脳神経外科領域にかかわる話題として，てんかん重積の最近のトピック，脳腫瘍関連てんかん，脳卒中後てんかんについて述べる．
