新しい頭部外傷ガイドライン

かつて頭部外傷診療は，初期診療から社会復帰まですべてを脳神経外科医が単独で担っていた時代もあったが，脳神経外科での専門診療はもちろんのこと，病院前救護，救急部門，手術部門，集中治療部門，リハビリテーション部門など診療局面が細分化し，さまざまな局面での診療内容が高度化したことにより，各職種のプロフェッショナルによる協働が時代の要請となった．職種や経験の異なるスタッフによる協働作業を円滑に進めるためのシステムが治療と管理の「標準化」であり，標準化の指針となるのが診療ガイドラインである．
本邦における頭部外傷の診療ガイドラインとしては，2000年に日本神経外傷学会（現・日本脳神経外傷学会）監修の『重症頭部外傷治療・管理のガイドライン』初版が発行された．その後も改訂して版を重ね，2019年10月には日本脳神経外科学会，日本脳神経外傷学会監修の『頭部外傷治療・管理のガイドライン第4版』として改訂し，新版が発行された．本稿ではその沿革と理念，主な改訂のポイント，今後の課題について概説する．
