脊髄硬膜内髄外腫瘍として認めた上衣腫の1例

Ⅰ．はじめに
　上衣腫は脊椎脊髄腫瘍の中で髄内腫瘍として最も頻度が高い腫瘍であり，原発性脊髄髄内腫瘍の1/4を占めるとされている^6,7)．硬膜内髄外腫瘍の形態をとるものは，終糸や馬尾を母地として発生する粘液乳頭状上衣腫であることが大多数である²⁾．しかし近年，硬膜内髄外腫瘍の形態で発生する上衣腫の報告が散見される．画像所見や発生形態，術中の所見，経過は粘液乳頭状上衣腫とは一線を画し，特に術前診断において注意を要する病態とされている．
　今回われわれは，胸腰椎移行部に硬膜内髄外腫瘍として発生した脊髄上衣腫の1例を経験した．文献的考察を加えて報告する．
