3 メタボリックシンドロームと動脈硬化の臨床検査

1988年にReaven1）は高血圧，耐糖能異常，高脂血症などを有する動脈硬化発症高リスク患者群に対しsyndrome Xと命名した．Kaplan2）によるdeadly quartet，DeFronzoら3）によるインスリン(insulin)抵抗性症候群，わが国では松沢ら4）が提唱した内臓脂肪症候群がほぼ同様の概念であると考えられている．このようなmultiple risk factor症候群の診断に関しては，個々の学会独自の診断基準が作られておりしばらくの間混乱を生じていたが，2005年のメタボリックシンドロームに関する国際会議において内臓脂肪の重要性が確認され，それに沿うような形でIDFやわが国から腹部肥満を必須項目とするメタボリックシンドロームの診断基準が発表された5）．AHA/NHLBIの基準では必須項目は設定されておらず，またADAやEASDはメタボリックシンドロームの存在に疑問を呈する見解を出していることから6，7），メタボリックシンドロームの診断における混乱はいまだ続いていると考えられる(表1)．
メタボリックシンドロームという疾患概念の確立は，高脂血症や高血圧，糖尿病といった動脈硬化性疾患発症の単独リスクを管理するよりも，それらの上流に存在する内臓脂肪の蓄積を是正することの重要性を示唆している．そのことは取りも直さずこれまでの個々の疾患を管理するという治療医学から，生活習慣の改善による内臓脂肪の減少を主体とした予防医学を重要視することを意味している(図1)．
