胆管細胞癌

サマリー

　胆管細胞癌は病理学的に胆管上皮由来の癌であり，原発性肝癌取扱い規約で肝内胆管癌(胆管細胞癌)と分類されている．最も有効な治療法は外科切除であり，その他の治療の有効性は確立していない．その発生におけるハイリスクグループが明らかでなく，また特徴的な臨床症状に乏しいため，発見された時点で既に切除困難な進行癌であることが少なくない．腫瘤形成型，胆管浸潤型，胆管内発育型の三つのタイプに分類されており，胆管内発育型の予後は良好であることが多いが，他のタイプでは満足のいく治療成績が得られていない．今後は外科治療と新規抗癌剤の組み合わせにより，予後が改善することを期待したい．
