膵管内乳頭粘液性腫瘍(IPMN)

サマリー

　膵管内乳頭粘液性腫瘍(intraductal papillary-mucinous neoplasia，IPMN)の特徴として，大量の粘液産生と，それによるVater乳頭部の開大および主膵管拡張，良好な予後などが挙げられる．男女比は2：1と男性に多く，平均年齢は男女ともに約65歳と高齢者に多く認められる．好発部位は膵頭部である．主膵管の拡張を主体とする主膵管型，膵管分枝の拡張を主体とする分枝型に大別される．分枝型に比較して主膵管型に悪性のものが多い．主要症状は上腹部痛，体重減少，易疲労感などであるが，症状のないものも比較的高頻度に認められる．分枝型の拡張分枝径30mm以上，壁在結節のある症例は悪性の頻度が増し，主膵管型とともに手術適応となる．粘液性囊胞腫瘍(mucinous cystic neoplasm，MCN)とは画像診断的あるいは肉眼的に鑑別するのがよい．IPMNは「ぶどうの房」，MCNは球形で厚い線維性被膜を有することから「夏みかん」と特徴づけられる．症例に合わせて最も適切な手術を選択する．IPMNはきちんとした手術をすれば治る可能性の高い疾患なので，縮小手術を行ったことによって腫瘍が再発し，患者を失うことは避けなくてはならない．
