神経芽腫

サマリー

　神経芽腫は交感神経冠(neural crest)細胞が分化，成熟する過程で腫瘍化したと考えられ，腫瘍発生母地は傍脊椎交感神経節や副腎髄質である．小児の腹部固形悪性腫瘍のなかでは最も多い腫瘍である．発見時には既に遠隔転移による多彩な臨床像を呈していることが多く，診断にはカテコールアミン代謝物である尿中バニルマンデル酸(vanillylmandelic acid，VMA)，ホモバニリン酸(homovanillic acid，HVA)や，血清中神経特異性エノル酵素(neuron specific enolase，NSE)の測定が有用である．わが国では1985年からマス・スクリーニングが行われた結果，早期発見例が増加したが^1），2004年をもっていったん休止された．腫瘍の分子生物学的特徴として，腫瘍染色体数が近2倍体(diploid)あるいは4倍体(tetraploid)であることや，MYCNの高増幅は予後不良因子であり^2），Trk-Aの高発現は予後良好因子の一つである^3，4）ことが挙げられる．治療は腫瘍の根治的摘除を原則とし，予後不良因子を有する腫瘍には術後化学療法を行う．進行例に対しては原発腫瘍摘除前後に抗腫瘍薬の多剤併用療法と局所放射線照射および造血幹細胞移植を加えた集学的治療を行う．遠隔転移を伴う進行例の予後は不良である．
