温式不規則抗体検出法

新しい知見

　輸血検査では，ABO式およびRh(D)の型判定に加え，溶血性輸血副作用や新生児溶血性疾患の原因となる温式不規則抗体の検出が重要である．不規則抗体の解析には，間接抗グロブリン試験が用いられる．そのため，厚生労働省の輸血療法実施に関する指針や輸血管理料算定の施設基準においても，間接抗グロブリン試験の院内施行が必須となっている．これまで，間接抗グロブリン試験は試験管法で行われてきた．しかし，臨床検査技師による輸血検査の24時間体制が普及している現在，検査者の熟練度に影響を受けない自動判定装置の必要性が高まり，カラム凝集法や赤血球膜固相法の日常検査への導入が進みつつある．
