6. 血管内皮機能の基礎と臨床

はじめに

　突然，「血管内皮細胞の機能が…」と言われても，いま一つピンとこない方が多いかもしれない．では，「動脈硬化の進展は血管内皮細胞の機能障害から始まる」と説明されたら，多少はイメージしやすくなるのではないだろうか．本稿では，次項の血管エコー法を用いた血管内皮機能の具体的な検査についての解説(血管内皮機能検査法)が理解しやすくなるように，基礎的な血管の構造や血管内皮細胞の機能から話を始め，血管内皮障害と動脈硬化の関係，さらには臨床的に血管内皮機能を評価することの意義や治療効果の判定および予後評価への応用などについて概説したい．
