2. ABPIの基礎と臨床

はじめに

　末梢動脈疾患(peripheral arterial disease，PAD)は，アテローム性動脈硬化により末梢動脈の狭窄や閉塞が起こり，これにより血流障害が生じる．また，PAD患者は複数の動脈硬化の危険因子および全身の動脈硬化性疾患をもっており，心血管イベントのリスクが著しく増大する．

　PADに対する最初の臨床評価は病歴聴取と検査である．PADの臨床所見の分類としてフォンテイン(Fontaine)分類やラザフォード(Rutherford)分類が用いられる．TASC(Inter-Society Consensus for the Management of Peripheral Arterial Disease)では，間歇性跛行の病歴でPADを疑うことはできるが，PADの有病率を過少評価する，と指摘している．これはPAD患者の2/3以上は無症候か，非定型的な下肢症状を有するためである．触診による動脈拍動の触知においては，足部動脈拍動が触知可能であれば，90％以上でPAD陰性であり，PADの除外診断が可能である．しかし，動脈拍動減弱や消失の拍動異常は，PADの有病率を過大評価させることも指摘されており，PADが疑われる患者すべてに対し客観的検査の必要性を訴えている．この検査法として非侵襲的血管検査である足関節上腕血圧比(ankle-brachial pressure index，ABPIもしくはankle-brachial index，ABI)の測定を推奨している^1）．
