可溶性フィブリンモノマー複合体の測定試薬の特性

はじめに

　近年，わが国においても血栓症の増加が指摘されている．血栓症はその診断が遅れると致命的になるため，早期診断による早期治療が重要である．血栓症の診断には病理診断や造影CTなどの画像診断が用いられるが，診断時には既に血栓症状が出現していることも多い．そこで，血栓形成をより早期に反映する検査として血栓マーカーが用いられる．血栓形成過程を考えれば，最も血栓に近いマーカーはフィブリンモノマー(fibrin monomer，FM)または可溶性フィブリン(soluble fibrin，SF)であろう．近年，わが国において3種類の新たなSFおよびFM複合体(fibrin monomer complex，FMC)の測定試薬が開発され，臨床応用が期待されている．しかし，測定値に試薬間差が生じ解釈に混乱を招く場合も少なくない．FMおよびSFの血中存在様式や血中動態が不明であることが混乱の原因の一つと思われる．本稿では，推定されるFM，SFの存在様式に基づき，SFおよびFMC各試薬での測定値の意味を解説したい．
