赤血球形態異常における最近の状況

新しい知見

　赤血球形態異常は，大きさ，染色性，形状，封入体などによって判別される．近年，日本検査血液学会の血液形態検査標準化小委員会によって標準化が進められているが，現状では，赤血球形態異常の臨床サイドへの報告は施設によって大きく異なり，臨床に対してどれだけ標準的な情報を提供できるかが課題となっている．また，赤血球の形態異常のなかには，破砕赤血球(schizocyte)のように臨床サイドに早急に報告を必要とする疾患や，病態と密接に関係しているものもあり，近年では血球分析装置を用いた破砕赤血球や有核赤血球(nucleated red blood cell，NRBC)の検出，網赤血球ヘモグロビン等量(reticulocyte hemoglobin equivalent，RET-He)/網状赤血球ヘモグロビン含量(reticulocyte hemoglobin content，CHr)の測定を試みた研究も多く報告されている.
