トロンボエラストグラム

新しい知見

　外傷患者における線溶亢進の診断にROTEM^(R)が有用であるという報告がLevratら^1）によってなされた．外傷患者23名にROTEM^(R)解析(組織因子を用いるEXTEMモード)とユーグロブリン溶解時間を行い，ROC(receiver operating characteristic)解析によりROTEM^(R)パラメーター(最大凝固塊堅性および凝固塊溶解指標)における線溶亢進基準値を決定した．この基準値を用いて，外傷患者87名を検討したところ5名に線溶亢進を認めた．ROTEM^(R)で線溶亢進を呈する患者は外傷重傷度スコア，死亡率ともに有意に高く(p＜0.05)，ROTEM^(R)が重症外傷患者の線溶亢進を早期に正確に検出しうる．
