心房細動に対するワルファリン療法

心房細動と抗凝固療法

　心房細動(atrial fibrillation，AF)は古くから知られた疾患であるが，昔はリウマチ性心臓弁膜症が多かったため，弁膜症に合併した心房細動が多かった．しかし，抗菌薬の発達とともにリウマチ熱は影を潜め，現在は心房細動と言えば非弁膜性心房細動(non-valvular atrial fibrillation，NVAF)のことを指すことが多くなった．NVAFの頻度は年齢とともに上昇するが^1），単に不整脈としての症状だけでなく，それに付随する合併症の脳卒中(脳塞栓)による社会的な影響が非常に大きい．現在では，脳梗塞の原因の30～40％が心原性の脳塞栓症と考えられており，心臓内にできた血栓はサイズが大きいため，比較的太い径の脳血管に塞栓症を生じやすく，大梗塞を生じるのが特徴である．

　いったん脳卒中を発症すれば，長い闘病生活やリハビリテーションが待っている．それにもかかわらず重い後遺症を残すことも多い．したがって，予防が大きな意味をもつ．
