ヘモグロビンA1c(HbA1c)は，慢性の血糖コントロール指標として糖尿病診療には欠かせない中心的な検査指標であり，DCCT(Diabetes Control and Complications Trial)1）やKumamoto Study2）において糖尿病合併症発症との関係が報告されて以来，糖尿病検査の主役となっている．一方，糖尿病の診断には慢性高血糖の確認が必要であるが，HbA1cは，その慢性の高血糖状態を反映する指標として，わが国では糖尿病実態調査や国民健康栄養調査などの疫学調査で活用されてきたことに加えて，糖尿病の補助診断としても早くから導入されてきた．HbA1cは2010年7月に改訂された糖尿病の新診断基準でも，血糖値と並ぶ判定指標として追加導入された．今後，HbA1c測定の重要性はよりいっそう高まると考えられるが，本稿ではHbA1cの臨床的意義と基準，HbA1c国際標準化を含めて概説する．
