癌化学療法中のB型肝炎ウイルスキャリアにおけるウイルス再活性化

はじめに

　B型肝炎ウイルス(hepatitis B virus，HBV)の再活性化は，癌化学療法・免疫抑制療法を行う患者において注意すべき問題であり，一部の症例においては劇症肝炎に至り，致死的な経過をたどることが報告されている．従来，hepatitis B surface antigen(HBs抗原)陽性例において多数報告されてきたが，最近，リツキシマブをはじめとする新規分子標的治療薬の導入によってHBs抗原陰性例からの再活性化が報告されるようになり，リスク分類を見直す必要性が出てきている．

　本稿では，癌化学療法中のHBV再活性化リスク分類および再活性化対策のポイントを概説し，その際に測定しておくべき重要な検査項目を提示する．
