心不全

サマリー

　わが国において心疾患は悪性新生物に次ぐ第2位の死亡原因であり，そのうちの約60%は心不全によるものといわれている．心不全とは心臓のポンプ機能の低下，それに起因する末梢循環障害に基づいて起こる症状の症候群であり，診断には胸部X線撮影，心電図，心エコー，採血検査などを用いる．治療は病態によって異なり，急性心不全と慢性心不全では治療法が異なる．治療原則は，①心臓ポンプ機能を規定する心拍数，前負荷，後負荷，収縮能のうち問題となる因子を調節する，②心不全の誘因を除去する，③長期予後を改善する治療法を選択する，④長期投与による合併症を予防する，⑤外科治療などの根治治療のタイミングを決める，などである．
