除菌前後のヘリコバクターピロリ感染胃病変と臨床検査

はじめに

　1983年，WarrenとMarshallが慢性胃炎患者の胃生検組織から分離培養に成功したHelicobacter pylori(H. pylori)は，その後胃十二指腸潰瘍^1，2），胃癌^3，4），胃悪性リンパ腫^5）をはじめとする上部消化管疾患や，特発性血小板減少性紫斑病(idiopathic thrombocytopenic purpura，ITP)^6）などの消化管以外の疾患との関連性が明らかにされてきた．

　H. pyloriは3～5μmの緩やかな右巻き螺旋状構造をもつグラム陰性桿菌であり，4～8本の有鞘性鞭毛を用いて活発な運動を行う．酸素濃度3～10％の微好気性環境でのみ発育し，発育環境が悪いと球状体(coccoid form)に変化する．H. pyloriの最大の特徴は菌体表層の強力なウレアーゼ活性により，胃液中の尿素を分解してアンモニアを生成し，胃酸を中和することで自らの菌体を保護していることである^7）．

　わが国でのH. pyloriの感染率は若年層で低く中高年層で高い傾向を示しており，10～20歳代では10％台にとどまっているものの，30歳代では30％台に上昇し，40歳代を境に60％以上と急激な増加傾向を示す^8）．H. pyloriの感染経路として飲用水を介した経口感染が考えられており，これらの傾向は第二次世界大戦後の1950年代に公共用水施設がわが国に導入され，上下水道の整備が進んだことと関連があると考えられている．
