新しい知見

　脳性ナトリウム利尿ペプチド(brain natriuretic peptide，BNP)とN末端プロBNP(N-terminal pro-BNP，NT-proBNP)は主に心室で合成・分泌される生化学的心機能・心不全マーカーである．近年，BNPとNT-proBNPは心不全診療だけではなく，急性冠症候群の診療にも役立つことが期待されている．NT-proBNPは腎機能低下の影響を強く受けるため，心腎連関マーカーに位置付けられている．しかし，現時点では両者の有用性は同等と考えられている．一方，心房性ナトリウム利尿ペプチド(atrial natriuretic peptide，ANP)は最初の生化学的心機能・心不全マーカーである．しかし，主に心房で合成・分泌されるため，心機能・心不全評価における精度はBNPやNT-proBNPより劣る．
