市中肺炎

はじめに

　市中肺炎は臨床医であれば誰もが遭遇するcommon diseaseである．臨床経過は年齢や基礎疾患の有無などの宿主側要因と耐性菌などの病原体側要因が密接に関係し，軽症から重症までさまざまである．病初期に適正な診断，治療がなされなければ死に至ることも少なくない^1）．わが国では2005年に日本呼吸器学会から「成人市中肺炎診療ガイドライン」が発表されており，エビデンスに基づいた検査や治療の実践を推奨している．本稿では市中肺炎の基本的な病態や肺炎診療における臨床検査値の変動，推移およびその解釈方法について述べる．
