軟部肉腫のKi-67発現

はじめに

　軟部肉腫はその発生母細胞や組織像が多種多様であり，発生頻度も低いため病理診断自体が容易ではない．免疫組織化学的検討が必須である．生物学的悪性度を推定することは治療を選択するうえで欠かせない．予後因子としては組織学的悪性度，腫瘍サイズ，転移の有無，発生深度，腫瘍進展範囲(コンパートメント)が重要視されている．このうち，組織学的悪性度に関してはTrojaniらの分類を基にしたFNCLCC(Fédération Nationale des Centres de Lutte Contre le Cancer)基準がWHO(World Health Organization)分類に採用されている^1，2）．それは腫瘍分化度，核分裂数，壊死，組織学的亜型から構成されている．核分裂数は0～9，10～19，20以上/10HPF(high power field)の3段階に分類されているが，観察者によってバラツキが大きい．これとは別に各種マーカーを用いて免疫組織化学的に腫瘍細胞の増殖活性を判定する試みが盛んである．判定が容易で，核分裂数とよく相関しているからである．
