子宮頸部腺癌

サマリー

子宮頸部腺癌は増加の傾向にあり，子宮頸癌の15～20%を占める．組織像が多彩でWHO分類では粘液性腺癌(内頸部型，腸型，悪性腺腫，絨毛腺管状乳頭腺癌)，類内膜腺癌などがある．初期病変は特有のコルポスコピー所見を示さないことが多く，病変の拡がりや浸潤の深さを正確に評価することが困難であり，診断のために積極的に子宮頸部円錐切除術が行われることが多い．初期病変で深さ3mm以下の症例では再発は稀である．浸潤腺癌は，扁平上皮癌に比べてリンパ節転移率が高く，放射線療法や化学療法に対する感受性が低く，予後は不良である．治療法は扁平上皮癌に準じるが，早期の腺癌に対しては手術療法が放射線療法よりも有効とされる．
