膵腺房細胞癌の細胞診

はじめに

　膵病変における細胞診断は，日常の細胞診業務のなかでは婦人科や泌尿器科領域のものに比べて一般的とは言えない．しかし，胆汁や膵液などの液状細胞診や腫瘍部からの穿刺あるいは擦過細胞診など，その種類や方法はさまざまで，そこから発生する腫瘍も多様であり，診断に苦慮する場合が少なくない．

　代表的な膵腫瘍は浸潤性膵管癌であり，本稿で取り上げる膵腺房細胞癌は発生頻度が低く，実際にその細胞診断を経験する機会は少ない．しかし，この癌の細胞像は特徴的であるため，それを把握しておけば正確な細胞診断に到達することは決して難しいことではない．そこで，本稿では膵腺房細胞癌を細胞診断するうえでのポイントや，この腫瘍の臨床病理学的特徴および鑑別診断について解説する．
