肝細胞癌とグリピカン3

はじめに

　近年画像診断が発達し，肝細胞癌(hepatocellular carcinoma，HCC)の診断精度は格段の進歩を遂げている．以前に比べてサイズの小さい腫瘍が見つかるようになり，腫瘍生検および手術で採取される検体では病理診断に難渋する症例が増えてきている．

　臨床的にはHCCの治療において手術のみならずラジオ波焼灼，腫瘍塞栓療法など選択肢が増え，それらの適応をみるため臨床から病理診断に要求する内容も変化しつつある．病理学的にはHCCは2010年に発刊したWHO分類から以前よりわが国で提唱されていた早期肝細胞癌の概念が認められるようになり，その分子生物学的特徴の理解も進歩している．

　本稿ではHCCの診断に有用性の高い分子マーカーであるグリピカン3(glypican-3，GPC3)について実際の組織診断との関連を中心に解説する．
